Medical Home Training and ResourcesUtah Integrated Services ProjectThe Utah Integrated Services Project's (UISP) overall goal was the systematic integration of community-based services for
children and youth with special health care needs (CYSHCN) and their families throughout Utah. Component Goals included:1)
Designing and implementing the leadership and infrastructure to accomplish and sustain this integration; 2) Using the Learning
Collaborative model, integrating the six core components into Utah community systems; 3) Provideing and continually enhancing
statewide resources and information to support the integration of the 6 Core Components into communities.
The Learning Collaborative consisted of five Learning Sessions (LS) that covered the concepts of medical home; early and continuous screening; and transition issues from birth into adulthood. Presentations and materials from the five Learning Sessions are provided in the sections below.
